you know..pretty much all this is covered between faq4, faq5 (first section) and upgradeXX.html. granted, upgradeXX.html is focused on "version jumpers", such as 4.9-release to 5.0-release, but I think most of your questions for incremental upgrades are covered if you UNDERSTAND what is there, too.
