I have this in clamd.conf:
StreamMaxLength 20M
ArchiveMaxFileSize 15M

Both default to 10 (in my installation, at least, which was from source
off their website), though, and unless your attachment is an archive and
ArchiveBlockMax is set, the second statement shouldn't hurt you.

There is another one:
MaxConnectionQueueLength 30 (default 15)
and
MaxThreads 20 (default 10)
that could be blocking your clamd's access to resources.

I assume there's sufficient space in your /var/tmp.
What does /var/log/clamd.log say?
You might want to set the "Debug" statement to get more info.

Not The Solution (tm) yet, but trying ..
