smtpd.conf(5) says:

"If the as parameter is specified, smtpd(8) will rewrite
the sender advertised in the SMTP session. "

The SMTP session is the "helo,mail from,rcpt to" part, so smtpd rewrites
what is specified in the "mail from" command. 

Than there is the "data" part, which contains the headers, one of them
the "From:"-header, which isn't changed. 

The "From:" header is set by the user agent (aka mail program).
