this is what I use

doas -u root scanimage --mode gray  -x215 -y297 --resolution 300dpi -B > 
fdsa.pnm

which works with the perms asis.  xsane only worked as root for me 
(across multple platforms/revs) so it's always been something that 
needed a lot of setup/takedown to use.
