The Altq mechanism only controls outbound traffic. IIRC the PF FAQ also mentions this. To obtain (partial) control over incoming external traffic, you can also place a queue on the internal interface. The internal interface sends out the incoming external traffic to your local machines. I mention "partial" as you cannot prevent certain parts of incoming traffic (e.g. UDP streams) from saturating your uplink with incoming external traffic.
