i really don't know where to begin.  what could have gone wrong with the
system?  is this something that indicates a hardware issue, or has something
happened to the 4.0 installation?

Sounds like a hardware error. Try replacing the dive, load it, and see what happens. If you can't boot the install CD, then I would say you have other issues.
