Over the past couple of years, I have seen this behavior come and go.
It seems to be pretty harmless, as far as I can tell, but I am curious about what causes this


an update, if wsconsctl keyboard.bell.volume variable it's != 0
it also beeps (and also not having xset b off seted on X). I think it's some
kind of  weird escape character captured by xterm or something (i'm not
a programmer myself).

as said before it's harmless but still curious.
