You had a hardware error so the rebuild was halted. This needs some prettying up but it comes down to a bad disk. The source disk has a read error so I suggest you saving everything off the disk before proceeding.
I will write a patch to terminate the rebuild and make bioctl reflect that better. Thanks for the report.
