So we're missing the usb detach events for some reason. That's not enough for me to solve the problem, but it's certainly critical information whoever may solve your problem will need. The obvious suggestion is to try a snapshot kernel and see if the same thing happens. I have a laptop where one port doesn't work right, not sure why, I just use the other ports.
