well that depends,  theoretically it would be possible to put one call on
hold and fetch the third incoming call, but chan_misdn does not yet support
that.

typically chan_misdn sends only a busy when it receives a busy from the
ToCalled Phone, but since 2 channels are already in use chan_misdn cannot
try to call a phone and ask it if it's busy.

unfortunately not yet, but i would say this is definitely an important
feature request, please put that on our bug tracker i will add an option
which will be called:

nochannel_reject_cause=

then you can set it to 17 which would be a busy. I will put this option into
0.4.0.
