Hi, as for the special case of loading *.mitk and *.hdr files, there already is a bug for the first issue (http://bugs.mitk.org/show_bug.cgi?id=8790 ) and I opened one for discussing the second ( http://bugs.mitk.org/show_bug.cgi?id=10340 ).
As for Matts original suggestion: I can see how a preferred setting for the loading dialogue might work if you are mainly working with one file type (so as not to confuse your users with the "additional files" they could, but should not, load). On the other hand the preferred setting for the saving dialogue is a bit more elaborate as not every preferred file type is reasonable for every kind of data ( e.g. vector image as vtk surface data or vice versa). If it is done as a preference page I could see it easily going out of hand and becoming very cluttered.
