Dear David, thanks for your inquiry. MITK is licensed under the BSD 3-Clause license and hence can be used as free open-source software even for commercial use as-is [1]. The German Cancer Research Center (DKFZ) does currently not offer any additional commercial licensing of MITK.
We took great care in depending only on license-compatible third-party libraries. There's of course Qt, which you may want to pay some extra attention to. While we are only depending the LGPL-parts of Qt, it is easy in MITK derived projects to also depend on more restrictively licensed parts that could potentially impose commercial licensing of Qt.
