You can use the PD patch as a starting point to modify the midi  
values, and then export them using a virtual midi port. If you have  
even a basic understanding of programming, you can easily change the  
behavior of leftWheelSpun and rightWheelSpun to return different  
values based on the status of other buttons.
The XCode project even automagically installs the new driver on each  
compile for you.

In the next version I'll organize the configurations into some kind of  
more dynamic system so the prefereces pane doesn't have to be modified  
when adding new subclasses of configuration. The golden ticket would  
be some sort of configuration editor, but that will probably never  
come, at least not from me.
