It should be simple. A line is both an inner member of the outer 
multipolygon, and the outer member of an inner multipolygon. So, 
basically you have two non-intersecting and non-overlapping 
multipolygons.

There are some lakes with islands with lakes in Finland. One is 
Partalansaari, if I remember correctly. You could also have small 
patches of forest in the middle of a field in the middle of a forest, or 
vice versa.
