Fix first few frames missing from H.264 starting with B-frames.
This fixes issue #89. It also fixes some frames repeating at the end of
the clip. Testing so far has not yet revealed any false positives for
the simple hueristic, and it potentially improves A/V sync.
Based on patch by Lutz Vieweg.
