At Megabyte we're using Trac internally and Mantis for clients. Trac is great but it seems more geared towards open source projects where it doesn't really matter who sees which tickets. Mantis can be configured so that issues are automatically hidden, which is needed for some of our projects. Mantis is in PHP, and Trac is in Python. I also used to use Scarab - which is very good in customisations, and is in Java. But its upgrade process was a bit flaky.
