I'm afraid this seems to be a problem in the Qt library then. I personally have had no problems on Vista when I still had access to a Vista machine (I've upgraded to XP since). Still, is there anything in C:\Documents and Settings\Duelle\.mnemosyne\error_log.txt? (Could also be that this directory is named differently in Vista) Feel free to send me your mem files. One just before you add the 3rd card, and one just after you try to add the third card. That way I can see if it's anything I can replicate on my machine, or whether it's really a Qt issue that is very specific to your setup. I assume you also tried to check the disk for errors?
