No, you are clear.  I understand the problem.

Gwern Branwen was right about the sharing of the *actual* single
deck.  The point of the application is that it learns how to respond
to your learning so sharing the deck from a single laptop not only
won't work.  It's counterproductive.

My recommendation is that every student update their own version of
the deck.  When they have added new cards then they can simply export
the category for which they are responsible and upload the .xml file
to a central place.  You can share any kind file via google docs so
it's a pretty ideal place to do it but there are other service as
well.  When the categories are updated then you will have to "check
them out" from where they are saved and add them to the deck on your
personal computer.  You can either rewrite over the cardiology.xml
each time you update (google likely notifies you when there are
changes to shared files) or agree on a scheme to keep it organized
(i.e. cardiology1.xml, cardiology2.xml, cardiology3.xml).  This is an
easy problem to resolve once everyone agrees on the plan.

The only thing that I can't account for are the duplicates.  When you
import from a category you will have all the previous duplicates from
what has already been created.  I don't know (because I haven't had
your problem) how the software handles those, whether it knows the
duplicates are there and does nothing with them or whether you have to
manually "clean duplicates" each time.  At any rate there is a tool to
deal with it so it's not really a problem.
