Actually, I had the mistaken impression from our previous thread that you used
these media files for other uses then Mnemosyne too, and then deleting that file
would give you problems elsewhere.

It's quite possible that I did give that impression. I can't recall. I intend to (but don't at present) regularly sync them to my phone so that I have a database of botanical images for reference. The most important factor for me (and one of the reasons I wasn't keen on anki) was to retain meaningful filenames and folder hierarchy. As the system for botanical nomenclature ( http://en.wikipedia.org/wiki/Biological_classification ) is hierarchical it lends itself perfectly to a hierarchical folder structure. I've a feeling that I originally used full sized images but now I resize them, with a further resizing that I do on-the-fly using a script depending on where I plan to review, adding a height=500 tag to each image card.

I could just as well delete all orphaned pictures upon card deletion then, no
need to mess with exta plugins or menu options.

I'd be happy with that behaviour as it fits in with my workflow. I delete 'bad' cards that I have erroneously attached to a poor quality image, so it would work out fine (for me). But the extra overhead of deleting the file manually (by pasting the filename from Ctrl-E) is not too heavy so I'm not worried either way.
