It's based on calendar date, but otherwise you have the idea right; you get pinged for a given card based on how long it's been since you've seen it and how well you rated your answer the last time you did see it (and some other factors, but that's about it).
In a nutshell, the higher your rating for a card/answer, the less often you see it so that stuff you miss often or are less comfortable with your answers you see more often. Of course, this requires some rigor and self-honesty in your ratings! =) There are other SRS programs out there that have a finer granularity; Anki for example will show you cards during the day when it thinks you should see them, mnemosyne is a once-per-day thing.
