Timothy Bourke tells me that the problem is due to Fast Format incorrectly replacing characters in the image paths. And indeed, all my problematic maps had an underscore in the path or file names. After I deleted that character and updated the img src= line of the associated cards, I can now view all maps without any problems, even with the Fast Format plugin enabled.
