Have you by any chance been using MnemoGogo? I recall that there was a similar issue a while ago, but I was under the impression that this had been fixed.

no longer treats this error as fatal, you'll only get a warning and will be able to continue normally.

Looking at the source, I can see why this could be triggered in rare
circumstances, especially on slower machines.
