I haven't seen anything like this myself, but let me know if you can find a pattern.

I don't know if this will make a difference to you but I have been
trying out new fonts, plus I have been attempting to work around the
'false latex' issue by deleting the latex media folder and restarting
mnemosyne - it keeps recurring.
