Mike, I of course don't know how you use Mnemosyne, so please forgive me if 
my suggestions are not relevant. But this comes to mind:

What grade do you generally give a new card when you first "learn" it? If 4 
or 5, you might try 2 instead. Then on subsequent tests of the same card, 
continue to give it a grade of 2 until you actually know it. Then give it 
gradually higher grades as you find you are remembering it even better. 
You'll know you are grading it too low when it appears too frequently or 
you feel it is now too easy.

The difficulty you are experiencing could also come from learning too many 
new cards at once, or if the form of the questions is too complex (e.g. 
expecting too much information as an answer). But I suspect you have 
already controlled for such factors.
