Initial fix in r7943
Call-ID affinity balancing algorithm by default (non-distributable).
Consistent hash
against specified header (default Call-ID) can be enabled optionally for
distributed LB.
Pluggable algorithm classes can be specified with "algorithmClass" load
balancer
property and will be loaded as long as the class is in the classpath.
