[MMouse]: japanese tour ep

i know this has been asked before but i wasnt listening then...
what does the japanese tour ep look like? i remember hearing there was no 
art, but what was the color of the sleeve and what did it say? does anyone 
happen to have any scans of it? what does the disc look like? you see im 
expecting a copy in the mail and would like to recreate package the best i 
could.
