I was able to reproduce the problem with apr/perlio. The problem is that when you run the test suite as root, Apache::Test runs the server under User: nobody, which explains the failure. The dir t/htdocs doesn't have a+wx perms. For now, please switch to your user, instead of root. And try again: I'm 100% sure that apr/perlio will work for you now.
