So it seems that there's some conflict between using mod_perl and mod_fastcgi, at least on Win32 (the autogenerated httpd.conf that Apache::Test generates and uses will enable any modules that are enabled in the system httpd.conf [except mod_perl], which, as you've found, is helpful in exposing problems like this). Unfortunately, I'm not familiar with fastcgi - perhaps someone on the list who's used it in combination with mod_perl can relate their experiences?
