Could this not be caused by a thread-safeness issue?
The downstream conn_rec is currently being kept in the configuration
record because there was opposition to putting it in the upstream
conn_rec. Is the configuration record thread safe? If not - then the
conn_rec of thread A will get hosed by thread B, which is probably when
the file handle disappears...?
