I have seen this issue. It seemed to be somehow related to the user importing a Contacts list from Outlook. The fields didn't map too well, especially Name fields. I exported her Contacts to an Outlook CSV file, cleaned it up in Excel, deleted all her WebMail contacts, and Imported. After that they were fine. Contact me offlist for details if necessary.
