My question is, what is the correct setup to prevent this from happening and keep the system redundant.

I've yet to reproduce this :\ Do you have a summary (or could link to the most relevent ml posts) on what you've done so far? I might give it a whack while I'm in the code this week.

Also, what is the behavior of fsck? Documentation is a bit lacking in this area and I would love to help fill in those blanks but I don't really know. Does it fix this kind of damage? Will it bump the number of replicas of old files from 2 to 3?

I don't know about bumping replicants down, but it'll bump replicants up if need be. The sources aren't too long if you want to find out exactly which cases it does handle.
