The discussions about fault finding reminds me of an anecdote told by a 
lecturer on a problem analysis course I attended over 40 years ago run by 
Kepner and Tregoe. He spoke of an American car manufacturer who were about to 
virtually terminate production of a particular model of car due to a problem 
with it for no apparent reason stopping.Once stationary it would start right up 
again. For example it would happen at any altitude, any weather condition. With 
a hot engine or cold and at all sorts of speeds they were desparate so they 
called in K& T who solved it. From what I remember of the method you should use 
it is ,When,How, What,  and Why although it was as I said over 40 years ago. 
Each time you answer for example "When" you again ask When,How What & Why until 
the solution is obvious. In the particular case above it turned out that the 
particular model showing the fault was traced back to a particular location 
where the guy making the final engine bay check stuck a pass label on the 
engine in such a way that under certain conditions the label was sucked up 
covering the carb air intake. Result car stopped.Label returned to original 
position and car would start again. 
 We were also told although not confirmed that they had a hand in the Appolo 13 
missions safe return.
