Summary: SQL: prepare output has unqualified table_name header

Initial Comment:
monetdb-> PREPARE select * from columns;
Exception in thread "main" java.lang.AssertionError: table_name is not fully 
qualified! (prepare)

Problem:

> % prepare,    prepare,        prepare # table_name             

all table names should include a schema here.  If none, ".prepare" should be 
used to qualify it.

prepare)
