I just fixed the last two problems with icc and on Windows.
See my respective checkins and/or
`cvs diff -r1.{1,2} src/modules/mal/pma.mx`
`cvs diff -r1.{2,3} src/modules/mal/pma.mx`
for details.

The first problem "warning: dereferencing type-punned pointer will break
strict-aliasing rules" is still open.
