Sounds like you have a number of issues. Starting with mongrel, what do the mongrel logs for the pids that have stopped running say ? Also check /var/log/system.log for monit messages.
It may be worth upgrading to monit 4.10.1, which includes a number of fixes for running monit under OSX.
