Hi, As Matthew and 'koko' have already well said, you need to install the perl-HTTP-Server-Simple (and possibly other Perl modules). Now, what really scares me is how you installed the Monitorix RPM package skipping such mandatory dependence(s).
Anyway, for best results use the repositories recommended in the Monitorix web site [1] and [2], and don't forget to download the most up to date version from them. I think that in this case (for CentOS 5.x), the later has a newer one. Regards.
