With all the super fixes in MDI, one problem is apparent. When a child is
maximized its the control icons are correctly merged into the menu-bar, however
when a maximized child is closed the icons disappear. They reappear on any
change of state, e.g. change the active child (e.g. Ctrl+F6 etc), or add a new
child.
Tested on SVN HEAD, Linux and Win32.
