Just go to the 'View' menu and click 'Configure Columns'. Simply press 'OK' and the columns are there. I think some way or the other the call to draw the columns doesn't get through initially. This could be either because something goes wrong when KeePass wants to draw the columns or that it simple doesn't in a way that works on mono.
