I have also noticed the issue when starting the debug, the app deploys, but never starts. Usually I deploy the app first then I hit debug. I have not had the problem since when doing it like that. It seemed to happen the most when I make code changes and hit debug. I don't remember it ever happening yet if I hit debug, but have not made any code changes.
