It looks like things broke on 4.1 because we can no longer determine package
version information; see:

(...though I do find it odd that in #6111 the app is apparently installed,
while in #6117 it isn't...)
What happens if you try installing it via MSBuild instead of the IDE?
cd Path\To\Project
MSBuild /t:Install ProjectName.csproj
