My understanding:

< 100 devices, you can use a normal account (and a normal monotouch).

Officially, for enterprise distro, EVEN IF IT'S < 100, you need an
enterprise account (and enterprise monotouch)

enterprise allows you to basically make your own appstore - you can
sign apps and they can go on any device which has your enterprise cert
on it, which can be deployed on a company-wide scale using Apple's
tools (iPhone Configuration Utility, I think, and also Lion Server).

To get an enterprise account, the company needs to be registered with
Dunn and Bradstreet, which is a mission if they are not a large
company (and also, possibly not do-able if you are in the UK???)

The 100 devices is per _account_, not per app. Might be an idea, if
you have < 100 devices, to get a new developer account for the
customer (or rather, get THEM to buy it)
