Ok, so "build adhoc / enterprise" was already ticked. I was still having code signing issues. I was creating distribution profiles but I couldn't get code signing to work at all. I think its related to the Identity in iPhone Bundle Signing. The distribution profile was appearing under a different identity and I don't know why.
So, I went back to Xcode, went to Provisioning Profiles and saw an option at the bottom called Automatic Device Provisioning. I clicked Refresh and it downloaded a *different* profile, but that one appears under my Identity in iPhone Bundle Signing. And that worked! That new profile contained all my testers, and they are now happily downloading and testing my app. Hope this helps someone else if they get stuck in the same area.
