Okay, so here's the ela. This has happend to me a few times. In all case its been when MonoDevelop barfs and dies. I seem to be reducing the number of these occurrences by being VERY mindful of the synchronization messages when i return to monodevelop after being in Xcode and don't even breath on it until its settled down.
When such a MD death occurs, the only way to get MD and Code properly back in sync is to close them both, re open Mono Develop, and reopen the GUI editor from MonoDevelop. This loses the last set of changes in XCode but gets the two back in sync and talking to each other. I hope that helps you iron out the issues.
