This greylisting software the ISPs are using to attempt to minimize Spam is a bad joke -- it's like the French building the Maginot line after World War I. The spammers are way ahead of them -- they constantly rotate which servers they hijack to use to send out their bulk mailings with, backdooring into them on legitimate accounts that have been left vulnerable by less-than-savvy users. By the time a server is identified as "sourcing spam" on the grey list, it is no longer doing so in most cases... the spammers have moved on to another server that is not yet on the grey list. So the owner of the grey-listed server has to jump through all kinds of hoops to get his server off the grey list. Then what happens? Right... a few weeks later the rotation comes back around to his server and it ends up sourcing spam for a few days again and getting greylisted again.
The end result is that virtually all of the spam is still getting through, but users are constantly finding legitimate messages they send being bounced back to them automatically "as spam" or are failing to receive legitimate messages which are being sent to them but which they never see because they are intercepted at the incoming server gate. The original plan was much better -- let each individual run their own spam filter and deal with the problem as best as they can.
