I am in no way qualified in the field of psychiatry or psychology, so I may be wrong, but I don't think there's consensus within the field that insanity is culturally defined. This is certainly one of the factors involved, but I think that in at least some circumstances there are also biological causes of various behavioral disorders that we call insanity. That is, it is often defects in brain functioning that results in problems.
