Something that I've found helpful is early on in the year doing a mini lesson 
on the types of questions the test makers give, and the types of answers they 
give. Then I have the students generate questions and answers that fir into 
these prototypes. I weave it into the discussion about what we are reading. It 
fits nicely into Lit cirlcles because generating questions is one of the tasks.
