In regards to the the phonics instruction, for students to apply, they need it in context to their reading. I would add to the posting about Daily 5 and recommend the CAFE' book by the same authors. In addition to comprehension strategies CAFE has it broken down into Comprehension, Accuracy, Fluency and Expanding Vocabulary. For each goal area there are strategy lessons. The phonics piece fits within the Accuracy (I can read the words) and Expanding Vocabulary. Showing how these strategies work during read aloud and following up when students are Reading to Self provides context for the skill application.
