if you know c++, you can insert your module directly into the moses 
code. You should look at the function
    TranslationOptionCollection::ProcessOneUnknownWord() line 201 in the 
cpp file
and change that to transliterate unknown words. At the moment, unknown 
words are simply copied from the source word as-is.

another method is to post-process the output from moses. The output from 
the standard error has a line
   BEST TRANSLATION: ...

An unknown word will have the following attached to it:
     |UNK|UNK|UNK
for example
     Timbuktu|UNK|UNK|UNK

You can search for this word 'Timbuktu' and use your transliteration 
model to do a better job of translating it
