Well this is very puzzling, but long story short,

TL;DR

netcam_url http://192.168.1.6/cgi-bin/mjpg/video.cgi

does the trick with V4.2.2

I've checked my config file backups to last October and I've always used
http://192.168.1.6/axis-cgi/jpg/image.cgi
so I'm not sure why or when it stopped working.

I found the working url by experimenting with ffplay after tosiara's 
suggestion then digging into the camera admin interface and working out 
the URL for the video stream there.

Seems to be working fine now and thanks for your help. The camera was 
heading for the bin so nice to get some more use out of it.
