I can be so absent minded, anyways there is at least one case I know of 
personally of a CA *loosing* documentation they were supplied, and when 
the person went to renew they had to ask him for additional copies...

I'm not sure how many CAs ask for that, most seem to just assume 
everything is still ok and renew regardless...
